Article 57

57.4. A voter shall record the followinginformation upon signing the signature sheet: 
• forename, surname, patronymic, date of birth;
• address of residence;
• ID or substitute document serial and batchnumber and date of issue; and
• date of signature.

Information about the voters required tobe recorded can be recorded on the signature sheets by a person who collectssignatures. Information mentioned above shall be recorded in handwriting andtheir non-dissemination distribution be ensured. 
57.5. While collecting signatures in support ofcandidates, each signature sheet must contain the following information aboutthe candidate: Signature sheets shall be prepared in accordance with Appendices#1,2,3 to this Code. 57.7. 
57.8. surname, forename, patronymic, birth date;


57.9. main working place or official duty,occupation (if it does not exist - type of activity).

Article 65. Collection Rule of Voters Signatures forEstablishing of a Campaign Group on Referendum

65.1.Signatures for creating a referendum campaign group shall be collected and madeofficial in accordance with Article 57 of this Code.
 

Signaturesheets shall be prepared in conformity with Appendix 4 of this Code.

65.2. If theinitiators establishing a referendum campaign group intend to conduct apre-referendum campaign in the entire territory of The Republic ofAzerbaijan, they shall have to collect a minimum of 40 thousand signatures onconsent to membership in the referendum campaign group in the territory of atleast 60 election constituencies.

In thiscase, the number of the potential members of a referendum
campaigngroup from one election constituency may not be less than
100 persons.
