Article 11.Collection of Citizens’ Signatures

1. The blank issued by the Central ElectoralCommittee for collecting citizens’ signatures must contain the followingapplication text:

I, a citizen of the Republic of Lithuania, certifythat I support the petition to call a referendum on (presenting the entire textof the resolution submitted for a referendum).

a) Row No.;

b) Citizen’s surname, name;

c) Citizen’s passport orPersonal identification Card number;

d) Birth date;

e) Residence place;

f) Signature;

g) Date.
